Police in the U.S. state of Ohio fatally shot a 12-year old boy who was holding a replica gun on a playground.

Police say officers shot the boy, identified as Tamir Rice, after he failed to raise his hands when ordered by police.

Officials say an officer fired twice after the boy pulled the fake weapon from his waistband.

Authorities say an investigation is underway.

Police were sent to a park after someone placed an emergency call to report a "juvenile" waving around what was described as a "probably fake" gun.

Officials say the replica was lacking the orange safety indicator on the weapon which indicates that it is a fake.
